To make sure patients understand their responsibilities when working with the health care providers at THE ENDOSCOPY CENTER.

Patients at The Endoscopy Center should understand and follow these responsibilities during their care:

• Provide complete and accurate information about your health, including medications (prescription, over-the-counter, and supplements) and any allergies or sensitivities.

• Ask questions if you do not understand instructions or explanations from providers or staff.

• Keep scheduled appointments and call the Center if you need to cancel.

• Follow the treatment plan recommended by your provider and take an active role in your care.

• Bring a responsible adult to stay during your procedure and drive you home afterward.

• Inform your provider if you have a living will, medical power of attorney, or other advance directive. If you have an advance directive, you must provide a copy for your medical record. However, advance directives are not honored at this Center, as all available medical resources will be used in the rare event of a complication.

• Pay any charges not covered by insurance and make payments for services as promptly as possible.

• Speak with your physician about the risks of refusing treatment, not following the treatment plan, or leaving against medical advice.

• Treat all health care providers, staff, and other patients with respect, and respect the property of others and the Center.

• Ask your health care provider what to expect regarding pain and pain management.

• Discuss pain relief options with your providers.

• Work with your doctor and nurse to create and evaluate a pain management plan.

• Request pain relief when pain begins and inform your provider if the pain is not relieved.

• Tell your provider about any concerns you have regarding pain medications.

Allow adequate time for your physician to review your protected health information (PHI) and make any needed corrections.
